Most users can simply enter their serial in the plug-ins's Settings->Register option, and do not need to use this form.
If you are using iLok licensing, fill out this form to fetch an iLok activation code.
If you are using Wave Arts licensing, but your machine is not connected to internet, you can use this form to fetch a license keyfile for offline registration. First find your Machine ID in the plug-in's Settings->Register Offline option, then select Wave Arts licensing method in form.
